As part of its efforts to monitor the educational progress of residents, the American Osteopathic Association requires the following two reports to be completed annually:
Residents
The "Resident's Annual Report" and the "Annual Paper Documentation Report" are required of each resident to be filled out and mailed to the AOCD office within 30 days of completion of each training year. Failure to meet this deadline may result in delayed evaluation and approval of the training year. Click on the buttons below to download these reports.
For Residents planning to lecture at the Annual Meeting or Midyear Meeting, the following two forms, "Resident Speaker Information" and "Faculty Disclosure Declaration", must be completed and returned to the AOCD office. Speakers will be chosen on a 'first come - first served' basis, so please return the forms as soon as possible.
Program Directors
Program Directors are required to annually complete a "Program Director's Report" and the "Core Competency Annual Forma" on each osteopathic physician in the program. This evaluation of performance of each resident must be accomplished and mailed to the AOCD office within 30 days of the completion of each training year. Failure to meet this deadline may result in delayed evaluation and approval of the resident's training year. Click on the buttons below to download these reports.
